how do i must download items and are they free to download?

You can find items in many of the threads around here (http://www.feral-heart.com/index.php?option=com_jfusion&Itemid=2&jfile=index.php&board=34.0).
They are free, unless you commission someone to make you one for money.
There is usually a readme in the downloads to help you install.

Yes, they are free to download.

I suggest downloading a software that allows you to unrar/unzip things, such as WinRAR..

Though, there are a lot of other ones out there as well, but these are the mainly used ones.

Adding on: If your having trouble with the items, here.
Put the item.cfg, in the main FeralHeart folder
then you put the items folder in the media folder ^.^
