Title: Can somebody give me a list of all the maps and how to get there?? xD

I I was wondering if somebody could give me a list, like for example, Lonely Cave> The grounds
Like which map leads to which, and if it leads to multiple just say it does? I'm so confused on finding places right now and I can't even find all the portals since it seems I don't see any portals on the map in the top right corner.
