Making New Items for FH 1.15

I have attempted to create a new item for FH v1.15 that will be private, and for some reason it shows in the cfg file and in-game, but the item itself fails to appear upon being added.

Any assistance from those whom have made packs for the new update?

Okay, got the mesh to work, now it's horribly off centered.
Looks like this:


I'm using Tailgates bandanna codes for positioning since it is a bandanna I am attempting to position:
