Hi! I need a preset made for me.

Sorry if the image is small, but I was hoping you could make something similar to this. The markings would be on both sides so the one pic is all you should need. If you need anything else, and accept the challenge, please let me know in the comments. I NEED THIS PRESET ASAP!!!!!!
