It could also be that you have anti-aliasing turned on. Make sure that in your video settings you have "Anti-Aliasing" turned to zero. You could also try lowering the resolution of the windows in your settings before trying to enter The Grounds.

Already done. If I were to turn the resolution lower, I wouldn't be able to see anything.

I have found that with some item packs, specific .meshes cause a crash and need to be replaced with some other .mesh that doesn't cause some odd random crash. Since it's The Grounds that causes you to crash, it's my best theory (since that's where all the people that wear all the items are!) for what is causing this crash.

To fix this, you will need to know what .mesh causes the crashing.
Your options are:
1. Go through the list of items and wear every single one until you crash and note what .mesh causes it;
2. Redirect stdout to a text file and examine it after a crash;

Personally I think 2. is faster (and more verbose), but a bit more complicated to pull off in Windows. You will need to start FH from cmd(dot)exe. So as follows: (ignoring the ' in commands, but do note ", also subsitute (dot) for a literal .)

1. Press windows + r for the run prompt, type in cmd(dot)exe
2. type in ' cd C:\FeralHeart ' or whatever the full path to your installation is.
3. Once cd'd (changedir'd) in (you can tell by the new path at the left of the prompt), type in ' FeralHeart.exe > "stdout.txt" 2>&1 '
that command will make a file in your FH directory with all the standard output so you can hopefully see why percisely you are crashing.

We're good guys, I somehow fixed it. I don't know how, but I did.
