How do I make portals in my map?

Oke so I finally managed to finish my own custom map.

Now is the problem I press *haves a portal* and then the positions of the Yaw
but when I go to the Cape of Distant my map appears and then I enter but I somewhere floating in the air
If go out of the game and refresch again with the character I went into the map I am in the map and i can run around and such things....what did I do wrong???

+ how can i make a new portel back to Cape Distant?
