how do i must download items and are they free to download?

You can find items in many of the threads around here.
They are free, unless you commission someone to make you one for money.
There is usually a readme in the downloads to help you install.

Yes, they are free to download.

I suggest downloading a software that allows you to unrar/unzip things, such as WinRAR..

Adding on: If your having trouble with the items, here.
Put the item.cfg, in the main FeralHeart folder
then you put the items folder in the media folder ^.^
