I need help. Chain collar seems to be out of place? [FIXED]

So I'm having a small game related problem. I have noticed that certain neck accessories are out of place? Like it seems that it is too big for my character to fit. I already checked my rendering subsystem but that doesn't seem like the problem. It happens most of the time after I tab out. Can anyone help me with this? (Edit) i also noticed that my character gets out of shape, the same as with the chain collar
